Your First Visit

Please arrive about 10 minutes early so that you have enough time to fill out a New Patient form. For your convenience, you can also print and fill out our New Patient Form to bring to your appointment.

New Patient Form

Initial Examination

Your dentist will review your medical and dental history and complete a comprehensive dental exam. Your exam will include a thorough examination of your gums, teeth, bite and jaw, an oral cancer screening, and any required X-rays.

Treatment Plan

Based on your history and examination, your dentist will develop a long-term dental treatment plan for you. Your dentist will also take the time to answer any questions you have about your dental health and treatment options.

Insurance & Payment Options

At My Care Dental, we'll help you navigate our options for payment and insurance claims so you can focus on achieving your dental goals. 

If you have any questions about your insurance coverage or the methods of payment that we accept, please do not hesitate to ask one of our administrative staff members.

Insurance Claims

We can direct bill your insurance company and our administrative team is happy to assist you in better understanding your insurance plan.

Payment Methods

The full treatment cost must be paid at the time of your appointment. We accept payment by cash, debit, and most major credit cards (including Visa, MasterCard, and American Express).
